Prostates get enlarged because of exposure to a rogue form of testosterone called dihydrotestosterone. Exposure to it causes prostate cells to grow. In both of these cell types, DHT binds to receptors which tell your prostate cells to grow. DHT is 10 times more potent than testosterone because it dissociates from the androgen receptor more slowly.
DHT is synthesized in the prostate from circulating testosterone by the action of the enzyme 5α-reductase, type 2. The most prescribed drug for enlarged prostates, finasteride (under the brand name Proscar), works by blocking the activity of 5α-reductase, type 2. Blocking that reduces DHT levels which can cause prostate enlargement if they are too high. Not only that but men taking finasteride were recently found to have 25% less chance of developing prostate cancers.
